All new and renewing applicants will be required to submit all of the required forms:

• Eye Exam Form   Exam date must be within 6 months of certification test date.
• Inspector Certification Portfolio  
• Inspector Certification Supervisor Affidavit  Please use if you are supervised by an individual
• Inspector Certification Self Supervisor Affidavit Please use if you are the owner or are self-supervised
• ACORD Certificate of Insurance Sample

In-House Inspector:  Inspector must provide certificate of company insurance showing a general liability policy of $1,000,000.00 minimum coverage.

Third-Party Professional applicant must provide a certificate of insurance showing Professional Liability of $1,000,000.00 minimum coverage.
